Why This Matters — Do Not Ignore This
Dear Veterans and Fellow Ex-Servicemen,
Namaskar. This is one of the most important things you need to check right now — before it is too late.
After you, your wife will receive your family pension. But if her name or date of birth in your PPO (Pension Payment Order) does not match her Aadhaar card or PAN card, the pension can be stopped or denied. Not only pension — her ECHS medical card can also be blocked. No matching details = no treatment.
This is not a small problem. Many veterans have faced this issue. The good news? It can be fixed, and this article will tell you exactly how — step by step.
The Golden Rule — Everything Must Match
There are four key documents. All of them must have the exact same name and date of birth of your wife:
- Your PPO (Pension Payment Order)
- Your Service Record / Discharge Book
- Your wife’s Aadhaar Card
- Your wife’s PAN Card
If even one of these is different from the others, you must get it corrected. The safest approach: make your service records match the Aadhaar card, because Aadhaar is now the primary identity document for all government purposes.
When Should You Apply for Correction?
You need to apply if any of the following situations apply to you:
- Your wife’s date of birth is not mentioned in your service record, PPO, or discharge book
- Only the year of birth or age is written, not the full date of birth
- The name or date of birth in your service record does not match her Aadhaar or PAN card
- The name is wrongly spelled or partially incorrect
Important Conditions Before You Apply
Before you start the process, keep these important rules in mind:
Regarding Date of Birth:
- The corrected date of birth must not make your wife a minor at the time of your marriage
- If marriage took place after 1978, wife’s age at the time of marriage must be 18 years or above
- If marriage took place before 1978, the minimum age at the time of marriage was 14 years
- If the difference between old and new date of birth is too large (say, 10 to 20 years), the Record Office may not accept it. Small corrections of 1 to 4 years are generally accepted
Documents You Need to Prepare
Here is the complete list of documents required. Read carefully and prepare each one:
Document 1 — Personal Application (in Triplicate)
Write a simple application addressed to your Record Office. It can be in Hindi, English, or any language — but preferably in English.
Your application must include:
- Army Number, Rank, and Full Name of the Ex-Serviceman (or name of the Widow if applicable)
- Full postal address with PIN code, mobile number, and email ID
- Address of the Record Office at the top left of the application
- Leave space at the bottom for the signature and stamp of the Zila Sainik Board (ZSB) officer
- Clearly state: “The date of birth / name of my wife was inadvertently recorded as [old details]. Her actual name and date of birth as per Aadhaar and PAN card is [correct details]. Necessary documents are enclosed. You are requested to rectify the same.”
- Attach a photocopy of all your personal details (PPO number, rank, date of retirement, etc.)
Make 3 copies of this application. All other documents need only 1 copy each.
Document 2 — Affidavit (on Non-Judicial Stamp Paper)
This is a legal declaration. Important points:
- Must be done on a ₹10 non-judicial stamp paper
- Must be attested by a First Class Magistrate — a Notary Public is NOT always accepted everywhere, so a First Class Magistrate is strongly recommended
- The affidavit must state: “My earlier date of birth as per service record was [old date]. My actual date of birth as per Aadhaar and PAN card is [correct date]. Both refer to the same person.”
- It must contain: Army Number, Rank, Name, Date of Retirement, Date of Death (if applicable), PPO Number, incorrect DOB, and correct DOB
- Submit the original affidavit, not a photocopy
- The affidavit must be in English
- If the applicant is a widow, her signature is mandatory on the affidavit
- The affidavit date must not be older than 6 months from the date of submission. Ideally, get it done within 7–10 days of submission
Document 3 — Aadhaar Card of Wife
- Provide a self-attested photocopy of wife’s Aadhaar card
- The Aadhaar must clearly show her date of birth
- If the date of birth is not printed on the Aadhaar card, first update the Aadhaar and then proceed
- The photocopy must be signed by the wife herself, not by you
Document 4 — PAN Card of Wife
- Provide a self-attested photocopy of wife’s PAN card
- PAN card always shows the date of birth — make sure it matches the Aadhaar
- Wife’s father’s name in the PAN card should also be correct
Document 5 — Birth Certificate (if available)
- If wife was born after 1985, a birth certificate is mandatory
- If born before 1985, it is not mandatory but helpful if available
- If the educational documents (school certificate, mark sheet, admit card) are correct, submit them
- Make sure the name and father’s name in these documents match the Aadhaar card
- If educational documents have errors too, it is better not to submit them as they may create more confusion
Document 6 — Marriage Certificate (if available)
- Must be issued by the Registrar of Marriage
- Should mention wife’s full name, date of birth, and father’s name
- Name and issuing authority details must be clearly printed
- If you have either the marriage certificate or the 10th Board certificate, that is sufficient. One of these two is very helpful
Document 7 — Non-Availability Certificate (if no birth or marriage certificate)
If you do not have a birth certificate or marriage certificate, get a non-availability certificate from your Gram Panchayat or Municipality. This certificate confirms that no birth/marriage record was registered at the time — which was very common in earlier decades.
Document 8 — Death Certificate of Ex-Serviceman (for widow cases only)
If the applicant is a widow applying on behalf of her late husband, include the death certificate of the ex-serviceman.
Document 9 — Self-Declaration Form
- Must be signed by the ex-serviceman (or widow in widow cases)
- Must have signatures of two ex-servicemen as witnesses, along with their army numbers, ranks, names, and addresses
- Must be counter-signed by the Zila Sainik Board (ZSB)
Document 10 — PDA Details
Details of the pension disbursing authority — which bank or office is currently paying the pension.
Document 11 — Delay Certificate (if applicable)
If the correction was not done for a long time, write a simple declaration explaining the delay. Mention genuine reasons such as not being aware of the mismatch, health issues, or any other valid reason.
Do You Need a Newspaper Advertisement?
This depends on what needs to be changed:

Step-by-Step Process — How to Submit
Follow these steps carefully:
Step 1 — Collect all documents listed above and self-attest each photocopy (wife must sign her own documents).
Step 2 — Visit your Zila Sainik Board (ZSB). Take all your documents. Get them checked by the clerk at the ZSB. If everything is in order, the ZSB officer will sign and stamp your application.
Step 3 — ZSB keeps one copy. The ZSB will retain one copy of your application (without other documents). They will return the remaining signed documents to you.
Step 4 — Send documents to Record Office by Speed Post or Registered Post. Write the Record Office address on the envelope and send all documents — do NOT use a courier. Keep photocopies of everything before sending.
Step 5 — Record Office issues a Part Order. Once the Record Office processes your application, they will issue a Part Order (any Part Order) for the name/DOB change. This Part Order will be sent to the Zila Sainik Board — not to your home directly.
Step 6 — Visit ZSB with your Discharge Book. Once you are informed that the Part Order has arrived, bring your Discharge Book to the ZSB. The rectification will be entered into your Discharge Book there.
Step 7 — Update SPARSH. Lodge a Service Request on the SPARSH portal to update the corrected date of birth. Track the request using the token number provided. If your pension account has not yet been migrated to SPARSH, write to PCDA (Pension) directly with a copy of the Part Order.

Before sending your application, make sure you have:

- ☐ Death Certificate of ESM (widow cases only)
- ☐ Self-Declaration Form with 2 witness signatures (counter-signed by ZSB)
- ☐ PDA details
- ☐ Delay Certificate (if long delay in applying)
- ☐ Newspaper advertisement (if name is being changed)
What Comes Next?
Once your wife’s name and date of birth are corrected in the service record and PPO, the same process can also be done for:
- Children’s name and date of birth
- Parents’ name and date of birth
This is especially important because ECHS card validation now requires all details to match. If your parents’ or children’s details in the ECHS card do not match the service record, their cards will be blocked and treatment will be denied. Watch out for the next video and article on this topic.
