CHANGE OR CORRECTION OF  NAME OF VETERANS IN PPO AND AF RECORDS  

There may be a requirement of correction or change in name in your official documents.  It has been seen that in Aadhar card it is very difficult to change or correction of your name.  All other civil documents are prrepared on the basis of the Aadhar Card.  So, it has been noticed that name should be everywhere identical and should be matched with Aadhar Card.  So, ESM may be in need to change his name in PPO and other Defence pension/service documents. In this article we will discus about the change of name of Air Force Personnel

DOCUMENTS REQUIRED TO CHANGE OR CORRECT OWN NAME IN AF RECORDS:-

1) PERSONAL APPLICATION. – 03 Copies
(a) Required in 3 original copies, signed by ESM. 
(b) Neatly hand-written /typed and printed in English. 
(c) On top LEFT side, Write To, then write Addressed of DAV. And below the address write (Through ZSB North 24 Parganas).
(d) On top RIGHT corner, Write From, then write  S.No, Rank, Name, Full Current Address, PIN Code, Mobile No and Email ID (If available).
e) Please leave enough space at the bottom of the application for stamp & signature of ZSB officer.
(f) Incorrect Name should match with Service Records and PPO. 
(g) Proposed correct Name should match with Adhaar Card and other supporting documents being given.
(h) Reason for change of Name must be given. 

Note-1: If you want to change Full Name, then submit documents with photos of both old and new names as proof. 

Note-2:  Name of Air Veteran cannot be changed/corrected after his death.

Note-3:  Incorrect name must be endorsed in IAF Records/PPO.

PUBLICATION OF  GOVT GAZETTE NOTIFICATION. 

(a) Compulsory document for WO, MWO, HFO, HFL. 
Note: Other ranks below WO may give Affidavit from 1st Class Judicial Magistrate, instead of Gazette Notification. 
(b)  Required in original in single copy and Self Attested. 
(c)  To be countersigned by Secretary, ZSB. 
(d)  Gazette can be either from State or Central Govt. 
(e) Both incorrect and Correct Names must be written.
(f) Should have your photo.

 AFFIDAVIT FOR CHANGE OR CORRECTION OF NAME .

(a) Affidavit from 1st Class Judicial/Executive Magistrate required.
(b)  Compulsory for JWO and below Ranks, if Govt Gazette is not given. 
(c) One passport size current colour photo of ESM to be pasted and signed accross the photo, avoiding face area.
(d) Left Hand Finger prints of all five fingers required.
(e) Service No, Rank, Name, Date of Discharge, PPO No are to be mentioned. 
(f) Both correct and wrong names are to be given and both names belong to the same person must be mentioned. 
(g) Reason for the change of DOB should be written. 
(h) Affidavit should not be more than 6 months old. 

NEWSPAPER ADVERTISEMENT FOR CHANGE OR CORRECTION OF NAME

(a) Advt is to be given in only ONE National Daily English/Hindi/Bengali (Local) Newspaper. 
(b) Rank, S.No, Name of ESM should be mentioned.
(c) Both incorrect and Correct Names and both belong to the same person should be mentioned. 
(d) Text of the Advt must be in English only. 
(e) Highlight the Advt and fold the Page to find and read easily. 

(f) Only the page in which Advt is printed to be folded and given. Other pages of the newspaper are not required. 
(f) To be submitted in original. No photocopy will be accepted. 
(g) Advt should NOT be more than 1 yr old.
(h) Ads in E-Paper/ Weekly papers/Online papers not acceptable.
(j) In case Advt is given other than English and Hindi, then write  the name of the Newspaper and Date of Publication on top of the Page and sign.

REQUIREMENT OF BIRTH CERTIFICATE (BC) AND OTHER DOCUMENTS

(a) BC may be given in single copy, if available. 
(b) Should be Self-Attested. 
6) MATRICULATION CERTIFICATE (MC).
Self-Attested, Single copy required.
7) ADHAAR CARD.
8) PAN CARD. 
(a) Mandatory Documents. 
(b) Both proposed name and DOB (in DDMMYYYY format) are to be correctly mentioned. 
(c) Self-attested photocopies are to be given in single copies. 
9) VOTER CARD.
10) PASSPORT.

If available, may be given. Must have proposed Name and DOB correctly written and self attested.

11) PPO/SPARSH PPO. 
(a) Mandatory Document.
(b) Copy of original PPO/ SPARSH PPO and also, all Corrigendum and EPPOs, if available, must be attached. 
(c) One self-attested photocopy of each PPO required.  

12) DISCHARGE BOOK. 
Only page having Service particulars may be given. Other pages not required.
13) PDA/BANK DETAILS FORM.
(a) This is a Mandatory Document.
(b) To be submitted in one original Copy. 
(c) PDA Form must be filled up and signed by ESM. 
(d) Counter signed by your PDA (Pension Disbursing Authority), which is your Bank Manager or DPDO or TO (Treasury Officer) etc.  
(e) If transferred to SPARSH then signature of PDA/Bank manager is not required. 

PROCESS OF APPLICATION:

STEP-1: Bring all above mentioned documents, along with all Originals in a folder to ZSB, 

STEP-2: After verification of your Application, it will be recommended by the Officer of ZSB.

STEP-3: Out of three copies of Personal Applications:-

(i) ONE copy to be sent to DAV by you, through Speed Post with all supporting documents. Do Not send by courier service. 

(ii) One copy only Application will be kept with ZSB, without any supporting documents.

(iii) One copy will be given to you as a personal copy. You should keep one full set documents for your future reference.

STEP-4: DAV will publish POR and upload in IAF Pensioner’s Portal. 

Note: DAV will NOT SEND any hard copies of PORs to you and ZSB by post. Please check whether your POR has been uploaded in IAF VETERANS website after 3/4 months.

Note: Please first Register Yourself in the website and then check if your POR copy is available in website. If available, then download and take a print. 

STEP-5: When POR is downloaded, visit ZSB, for endorsement in your Discharge Books. 

NOTE-1: If possible, bring printout of POR, as per page size of Discharge Book, for neat pasting and attestation. 

NOTE-2: YOU DO NOT NEED TO APPLY AGAIN TO DAV TO GET CORRIGENDUM PPO, WITH CORRECT NAME OF ESM. 

NOTE-3: DAV WILL TAKE UP CASE WITH:-

JCDA( AF) (for Post-2006 Retirees) 
OR
PCDA(P) (for Pre-2006 Retirees),

TO AMEND YOUR NAME IN SPARSH PPO FOR YOU, AFTER YOUR PENSION IS MIGRATED TO SPARSH. 

NOTE-4: If you have not migrated yet, please wait for transfer to SPARSH. 

STEP-6: IF NOT AMENDED BY THE DAV, YOU MAY AMEND IT YOURSELF BY LOGGING IN AND USING EDIT BUTTON IN SPARSH PORTAL.

STEP-7: SPARSH will get your request verified from DAV and update in your SPARSH PPO. You will be issued with a docket number to track your request. 

After your SPARSH PPO is amended with your correct Name, there will be no problem with your pension.
